Empower older people for self-management on health

The “Jockey Club Community eHealth Care Project” was launched in 2016. The Project is the first territory-wide elderly care support project which integrates health management technology, community care and professional support with a view to promote preventive healthcare among the elderly and empower them in health management.

 

Under the Project, eHealth stations have been set up at the elderly centres, where elderly participants can conduct regular health measurement of blood pressure, blood glucose and weight, and their health data are monitored and followed up by a multi-disciplinary team (nurses, health workers, social workers) such as providing nursing caring calls and regular outreach visits. The Project has also applied big data analytics to examine possible health risks of the elderly and find out the association between daily health readings and the long term health-status.
